Financial Responsibility

We believe in transparent, predictable pricing. This agreement explains how billing works at Attunio — what your insurance covers, what you owe, and how and when payments are collected — so there are no surprises.

1. Overview

This Financial Responsibility Agreement explains your obligations for payment of services provided through Attunio. By booking a visit or receiving care, you acknowledge that you are financially responsible for all charges, whether paid by insurance or directly by you.

2. Insurance and eligibility

When you provide insurance information, we verify your eligibility and benefits before your visit. Eligibility verification is not a guarantee of payment. Your insurer makes the final determination of coverage after a claim is submitted.

  • You are responsible for providing accurate, up-to-date insurance information.
  • You are responsible for understanding your plan's coverage, network status, and any referral or pre-authorization requirements.
  • If your plan denies a claim or considers a service non-covered, you are responsible for the full amount.

3. Copays, deductibles, and coinsurance

You are responsible for any cost-sharing amounts defined by your insurance plan, including:

  • Copays: a fixed amount due at the time of service.
  • Deductibles: the amount you pay before your insurance begins to contribute.
  • Coinsurance: the percentage of costs you share after meeting your deductible.

We collect estimated patient responsibility at or before the time of service. Any difference between the estimate and the final adjudicated amount will be billed or refunded after your claim is processed.

4. Self-pay rates

If you do not use insurance, or your service is not covered, self-pay rates apply. Current self-pay pricing is displayed before you confirm a booking. Self-pay charges are due at the time of service.

5. Payment methods and timing

We accept major credit and debit cards and eligible HSA/FSA cards. A valid payment method is required on file to book a visit. You authorize Attunio to charge your payment method for:

  • Patient responsibility amounts estimated at the time of service.
  • Balances remaining after insurance adjudication.
  • Applicable no-show or late-cancellation fees.

6. Cancellations and no-shows

Appointments cancelled with less than 24 hours' notice, or missed without notice, may incur a fee. The applicable fee is disclosed at booking and is not reimbursable by insurance.

7. Billing disputes

If you believe a charge is incorrect, contact our billing team within 60 days of the statement date at billing@attunio.co. We will review and respond promptly. Undisputed balances remain due while a dispute is investigated.

8. Past-due balances

Balances not paid within 30 days of billing may be considered past due. We may pause non-urgent services until your balance is resolved. We will always work with you on a reasonable payment arrangement — contact us before a balance becomes past due.
